NCMHCE exam facts: registration, timing, retakes

Under NBCC's state-licensure handbook, booking the NCMHCE starts with permission, not with Pearson VUE: approval from your state board (state route) or an NCC/CCMHC application (national route). Texas and Florida are exceptions, since their boards let you register without prior approval. After that you pick a seat at a center or online. The table lists every rule with its source, and the sections below explain registration, exam-day timing, retakes, the 2022 redesign and the form that replaces this one in 2027.

How registration works

There are two routes, and the order of steps differs:

  1. State route. You apply to your licensing board. Once the board approves you, NBCC's affiliate CCE sends an authorization and you schedule with Pearson VUE inside the window your board allows. Board forms and fees: license requirements by state.
  2. National route. You apply to NBCC for a national certification that uses this exam (NCC or CCMHC), then schedule the same way. Requirements: NCC (National Certified Counselor).

If you need accommodations, settle them before you pick between a center and OnVUE. Moving the appointment later costs the reschedule fee shown in the table.

Exam-day timing

The appointment runs 255 minutes, of which the questions get 225. The table breaks down the remaining half hour; plan to need only the one scheduled break.

Inside a case you move forward only: you can change answers within the current part of a case; once you move to the next part, the earlier parts are locked. On OnVUE the first five cases also close after the scheduled break. Settle every flagged question before you click on.

With 11 cases in 225 minutes, you have about 20 minutes per case. A long intake earns a full read; skimming it to save a minute usually costs more on the questions.

Scores and results

The passing score is set per form, not a percentage. You get an unofficial score report as soon as you finish: from the proctor at a test center, or on your Pearson dashboard online. The official result goes to your board. How the cut is set, what the 2027 scale means and the only published pass-rate figures: NCMHCE passing score.

Retakes

The wait is 30 days from the test date, and each attempt is registered and paid for from scratch. How many attempts you get is up to your board; the table lists the national-route limit.

Where your board grants an approval window, a retake has to fit inside it; once the window closes, you ask the board for a new approval letter first. An accepted appeal earns a free retake, not a higher score. Budget a retake before you need one: NCMHCE cost.

The 2027 form

From July 1, 2027 the exam is 10 case studies with 130–150 questions, 30–50 of them unscored field-test items; an optional 15-minute break halfway, and answers can be reviewed only within the current half. On top of any lock inside a case, the first half closes once you submit it. NBCC published the new specifications in 2026 together with a crosswalk from today's domains to the new ones. How the five current domains map:

  • Professional Practice and Ethics → mostly Legal and Ethical Compliance; supervision and consultation move to Professional Development and Counselor Self-Awareness, informed consent to Intake and Assessment.
  • Intake, Assessment, and Diagnosis → Intake and Assessment.
  • Treatment Planning → Treatment Planning and Continuity of Care; documentation and coordination with other providers move to the new Indirect Client Care domain.
  • Counseling Skills and Interventions → Provision of Counseling Interventions.
  • Core Counseling Attributes → self-awareness goes to Professional Development and Counselor Self-Awareness; the attributes themselves are tested as intervention skills.

The crosswalk also lists tasks that are new to the outline, among them the use of AI in practice, telehealth consent and platforms, the Counseling Compact, electronic health records and liability insurance.

The 2022 redesign

The current case format replaced, in 2022, an exam built from clinical simulations where you chose which information to gather and lost points for poor choices. Study material written for that version still circulates; if it mentions information gathering, decision making sections or negative scores, it describes an exam that no longer exists.

Areas of clinical focus

The outline's third domain is a list of 50 presenting concerns (items A to AX in the October 2025 content outline). It carries no questions of its own; NBCC samples it through the diagnoses and problems written into each form's cases. Grouped, the list covers:

  • Mood, anxiety and thought: depression and hopelessness, worry, fear and panic, obsessive and intrusive thoughts, rumination, attention problems, hallucinations, emotional dysregulation.
  • Risk and trauma: suicidal thoughts and behavior, trauma, child abuse, family violence, partner violence, bullying.
  • Body and habits: sleep, eating, substance use, process addictions such as gambling, sexual functioning, physical illness and injury, terminal illness.
  • Life stage and loss: grief, end-of-life issues, aging, retirement, child development, separation from caregivers, adoption.
  • Couples and families: divorce, remarriage, blended families, co-parenting conflict, communication problems, dating.
  • Identity, culture and meaning: gender identity, racism and discrimination, cultural adjustment, religious and spiritual conflicts.
  • Daily life: caregiving, financial stress, career, loneliness, stress management, intellectual functioning.

The 2027 specifications drop it as a domain and keep a similar list of presenting concerns outside the blueprint.

Questions people ask

How long does my authorization last?

Where your board issues an approval, it sets the window, up to six months. Book an early date: a retake after the 30-day wait has to fit inside the same window, or you need a new approval letter.

Which form will I get if I test in spring 2027?

Today's form. NBCC switches on July 1, 2027, and the version you take depends on your test date, not on when you registered.

Can I bring a DSM or notes?

No. The exam is closed-book at the center and online. Everything you need to know about the client is in the case.
